I´m not sure, if that´s the right form, probably they give you one anyway at the POLÍCIA FEDERAL when you file for your PERMANÊNCIA DEFINITIVA.

On the PF website (www.dpf.gov.br) there is a 4 page form (DPF1344) for download, download it to get an idea what kind of documents you need and how to fill out the application.

Arriving at the PF they probably won´t accept the downloaded form and give you one of their own anyway. But this way you arrive at the PF prepared. Wich may avoid a trip or two for nothing to the PF.

I recomend to visit them as soon as possible to verify the details. Such as, if it´s possible to file for a change of status, from your current visa to permanent. Or check if you can only file for PERMANÊNCIA DEFINITIVA. That could make a difference in the documents you have to present.

But it´s easier to check this in a personal chat at the PF.

About your CNH you have to go to the local DETRAN and file for renewal, have to make a medical exam and within 1 week you can pick up you new CNH. Fee, about R$ 65,-

The same for the CIE (RNE) go to the POLÍCIA FEDERAL and file for renewal, it´ll probably take up to 1 year or more for them to issue the new CIE at the moment. Fee, about R$ 124,-

But there is no problem about renewing thoose two documents while you in the middle of status change or applying for the permanent visa.
